Do I need to bring anything to my assessment?

The following documents are required for having an assessment:

• A copy of your medical evidence or diagnostic report.
• A copy of your funding body eligibility letter.

We recommend that you send these documents to us in advance of your assessment, as your funding body does not send us this information.
